Job summary

HarmonyCares Medical Group in the United States seeks a Nurse Practitioner to deliver patient-centered, high-quality care in patients' homes, focusing on those with complex health needs. The NP will work within a physician-led, team-based model, prioritizing time with patients over volume.

The role targets clinicians with geriatric or home health experience and requires state NP licensure, DEA eligibility, and board certification.

Qualifications

• Direct experience in home health, geriatrics, hospice and primary care.
• Clinicians should focus on patient care over volume and take time to address patient needs.
• Active NP license in state of practice, DEA/CPR-BLS and board certification required.

Responsibilities

• Provide primary care to high-acuity patients in their homes, Monday–Friday.
• Collaborate with assistants in select markets to support appointments.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as an NP in the State where HarmonyCares Medical Group office is located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record
