Job summary

HarmonyCares is seeking a skilled Nurse Practitioner to deliver in-home primary care to patients with complex needs. This role emphasizes patient-centered care, thoughtful assessment, and collaborative teamwork to support home-based services.

You will treat high-acuity patients in home settings, maintain required licenses, and participate in ongoing professional development. A strong focus on quality outcomes and care coordination is essential.

Qualifications

• Must be licensed as a Nurse Practitioner in the state of practice.
• Federal DEA license or eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification required.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high acuity patients in patients' homes.
• Deliver patient-centered care with time to address individual needs, not volume.
• Collaborate with care teams to optimize home-based care delivery.

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
