Job summary

Care Med in Ohio is seeking a Nurse Practitioner to provide primary and preventative care for residents in long-term care facilities. You will manage residents with acute changes, participate in care planning, and collaborate with the interdisciplinary team.

The role requires OH NP license, board certification, DEA licensure, and experience in geriatric care or long-term care settings. EMR proficiency and ability to coordinate with facility staff are essential, with a comprehensive benefits

Qualifications

Current, unrestricted NP license in OH.
Board Certification in nursing practice.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience in long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team.

Responsibilities

Provide primary and preventative care for long-term care patients in skilled nursing facilities.
Manage residents with acute changes in condition.
Advanced care planning and chronic care management.
Prescribe medications, including controlled substances per OH law.
Order, review and follow-up with treatments and diagnostic testing.
Communicate with nursing home staff about changes in condition.
Collaborate with primary care physician.
