Qualifications
- Active, unrestricted NP license in Kentucky.
- Board Certification in NP specialty.
- DEA licensure to prescribe controlled substances.
- Clinical background in adult, family or geriatric care.
- Experience in long-term care settings.
- Ability to work with an EMR.
Responsibilities
- Provide primary and preventive care for long-term care residents.
- Manage residents with acute changes in condition.
- Advanced care planning and chronic care management.
- Prescribe medications, including controlled substances per Kentucky law.
- Order, review and follow up on treatments and diagnostics.
- Communicate with nursing home staff on changes in condition and care plan.
- Collaborate with primary care physicians.
