Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to deliver primary and preventive care to residents in skilled nursing facilities. You will work as part of an interdisciplinary team to maintain high-quality care.

Responsibilities include managing acute changes, advanced care planning, chronic disease management, prescribing medications per Ohio law, coordinating with primary care physicians, and ensuring appropriate treatments are ordered and followed.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus

Responsibilities

• Provide primary and preventive care for long-term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic disease management.
• Prescribe medications, including controlled substances per Ohio law.
• Order, review and follow up on treatments and diagnostics.
• Communicate with nursing home staff about changes in condition or care plan.
• Collaborate with primary care physician to coordinate patient care.
• Work within an interdisciplinary care team to ensure high-quality care.
