Job summary

CareMed Health Solutions is seeking a licensed Nurse Practitioner to provide primary and preventive care for residents living in long-term care facilities. You will manage acute changes in condition, engage in advanced care planning, and collaborate with an interdisciplinary team to coordinate patient care.

Responsibilities include prescribing medications under Ohio law, ordering tests, and communicating regularly with facility staff.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board certification and DEA licensure.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care and EMR usage.
• Ability to work with an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long-term care residents.
