Job summary

CareMed Health Solutions seeks a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities in Ohio. You will manage acute changes, perform advanced care planning, and prescribe medications following OH law, while coordinating with nursing home staff and collaborating with the primary care physician.

Ideal candidates hold an Ohio NP license, board certification, DEA licensure, and have experience with geriatric/long-term care patients, including EMR

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long-term care patients in skilled nursing facilities.
• Manage Residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per OH law.
• Order, review and follow up on treatments and diagnostic testing.
• Communicate with nursing home staff regarding changes in condition or care plan.
• Collaborate with primary care physician.
