Job summary

CareMed Health Solutions in Ohio seeks a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities, including managing acute changes and coordinating care plans with an interdisciplinary team.

Applicants must hold an unrestricted Ohio NP license, board certification, and a DEA license. Long-term care experience and EMR proficiency are preferred. A competitive salary and benefits accompany the role.

Qualifications

Current, unrestricted NP license in Ohio.
Board Certification required.
DEA licensure required.
Clinical background in adult, family or geriatric specialties.
Experience in a long-term care setting.
Proficient with EMR systems.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team.
Psych experience a plus.

Responsibilities

Provide primary and preventative care for long term care patients in skilled nursing facilities.
Manage residents with acute changes in condition.
Advanced care planning and chronic care management.
Prescribe medications in accordance with OH law, including controlled substances.
Order, review and follow up on treatments and diagnostic testing.
Communicate with nursing home staff on changes in condition or care plans.
Collaborate with the primary care physician.
