CareMed Health Solutions is seeking a Nurse Practitioner to deliver primary and preventive care for residents in long-term care facilities. You will manage acute changes in condition, coordinate advanced care planning, and ensure appropriate treatment and testing within KY law.

Competenze

• Current, unrestricted NP license in the state of KY.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.

Mansioni

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage Residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per KY law.
• Order, review and follow up on treatments and diagnostics.
• Communicate with nursing home staff regularly about condition changes.
