Job summary

Responsibilities

• Attend daily morning meetings when in facilities.
• Work within prescriptive authority agreements with attending physicians.
• Provide provider-level care delivery and preventive care for ISNP Plan members.
• Respond to compliance requests in a timely manner.
• Enter data daily into the designated EMR systems.
• Participate in ongoing staff education and quality improvement.
• Communicate professionally with families and facility staff.
