Job summary

Optum Home & Community Care is expanding its HouseCalls program in the United States. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct annual in-home health assessments for Medicare Advantage and other plan members.

This is a non-prescribing, field-based role focused on improving outcomes through education and collaboration with PCPs. We offer flexible scheduling (8am–8pm, 7 days a week), no on-call, no weekends, and no holidays.

Qualifications

• Active ANCC or AANP certification for NP or PA as applicable.
• Active and unrestricted license in the practicing state.
• Valid driver's license with reliable transportation for home visits.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and treatment planning.
• Educate members on disease processes, medications, and self-management.
• Coordinate with PCPs to close care gaps and address urgent situations.
