Optum Home & Community Care, part of the Optum family, is expanding its HouseCalls program offering in-home assessments by Advanced Practice Clinicians (NP/PA) for Medicare Advantage members. The role focuses on education, gap closure, and collaboration with primary care, with no prescribing responsibilities.

Flexible scheduling is offered (8am–8pm over 7 days), up to 40 hours/week. This field-based position emphasizes quality outcomes, social determinants of health, and safe home visit

Qualifications

• Active ANCC or AANP certification for NP track.
• Current NCCPA certification and state licensure for PA track.
• Active, unrestricted licensure in practicing state without disciplinary actions.
• Active driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If applicable, eligibility to practice independently with required approvals.

Responsabilités

• Conduct comprehensive in-home assessments including history, vitals, and exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to close care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
