Job summary

Elara Caring is hiring a Nurse Practitioner to provide high-quality care in patients’ homes, helping to reduce hospitalizations and support families on their health journeys.

The role requires a Masters in Nursing, NP licensure, and active DEA. Travel up to 50% is expected to support patient needs. You will join a mission-driven team that values education, training, and collaboration with the Medical Director and branch leadership.

Qualifications

• Masters Degree in Nursing and certification through a national Nurse Practitioner Licensing board.
• 3+ years clinical nursing experience
• 1+ year post-graduate clinical nursing experience in the hospice and palliative care field.
• Prior experience in Family Medicine, Geriatrics and psychiatry preferred.
• Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.
• Active DEA license.

Responsibilities

• Ensures activities align with the vision of Elara Caring’s board, executive team, and Hospice leadership.
• Prevent hospitalizations by meeting patient and family needs in the home.
• Prepare and maintain accurate patient records for sound medical practice and reimbursement.
