Job summary

Heart to Heart Hospice is seeking a Nurse Practitioner to provide advanced clinical care in a hospice setting in Fort Worth, TX. You will educate patients and families, contribute to care planning, and collaborate with the hospice medical team to deliver compassionate end-of-life care.

Applicants must hold RN and APN licenses, be a graduate of a Baccalaureate School of Nursing, and have reliable transportation. Two years of APN experience in hospice or home health is preferred.

Qualifications

• Knowledge of pharmacology, palliative care, and end-of-life care.

Responsibilities

• Consult with patients and families about disease, self-care, and end-of-life care.
• Assist in IDG development of plan of care to ensure quality care.
• Collaborate with physicians on palliative care plans.
• Inform physicians of changes and monitor patient responses.
• Ensure timely documentation and compliance with laws and regulations.
• Perform other duties as required.
