Community Healthcare of Texas is actively recruiting an Advanced Practitioner Nurse to deliver palliative and hospice care in Fort Worth, TX. Under Medical Director supervision, you will collaborate with the care team to manage patient care and ensure quality outcomes.

The role requires Texas APN licensure, 3+ years nursing experience, strong assessment and communication skills, and proficiency with Microsoft Office. Competitive benefits accompany full-time hours.

Qualifications

• Registered Nurse with APN licensure in Texas and applicable certifications.
• 3+ years of nursing experience, preferably at APN level.
• Knowledge of hospice and palliative care principles and protocols.
• Proficiency with computer systems and Microsoft Office suite.

Responsabilités

• Documents medical history, hospice eligibility, disease progression and medications per standards.
• Consults Medical Director or designee when plan of care falls outside standard practice.
• Updates and coordinates plan of care with the interdisciplinary team (IDT).
• Ensures patient symptom control through IDT collaboration.
• Documents care in the patient’s medical record and evidences disease progression.
• Attends physician meetings and other required meetings.
• Builds relationships with staff and community referral sources to support program growth.
• Participates in QA/PI processes as required.
• Performs duties within applicable regulations and agency policies.
