Job summary

Community Healthcare of Texas is seeking an Advanced Practitioner Nurse to deliver hospice and palliative care under the supervision of the Medical Director. This full-time role in Fort Worth, TX involves collaboration with the care team to provide comprehensive patient- and family-centered services.

The role requires TX APN licensure, strong knowledge of hospice care, and experience with interdisciplinary teams. Competitive pay and benefits are provided, with standard office hours.

Qualifications

• Registered Nurse duly licensed to practice as an Advanced Practice Nurse in Texas.
• APN licensure with TX State Board of Nurse Examiners provisions is required.

Responsibilities

• Document medical history, hospice eligibility, disease progress, medications, and related data in patient records; communicate findings to Medical Director.
• Consult with Medical Director or designee for plans outside standardized practice; coordinate specialty consults as needed.
• Review, follow, and update the written plan of care with the IDT.
• Ensure symptom control through collaboration with IDT members.
• Document care provided and progression of end-stage disease in the clinical record.
• Attend physicians' meetings and other required gatherings.
• Establish and nurture relationships with staff and community referral sources.
• Participate in QA/PI processes and keep within regulatory requirements.
• Perform all duties within local, state, and federal regulations for palliative and hospice care.
• Perform other related duties as directed.
