Job summary

Heart to Heart Hospice in Temple, TX is seeking a Nurse Practitioner to deliver compassionate, patient-centered end-of-life care. You will work with the care team to educate patients and families about disease, treatment options, and self-care at life’s end.

If you hold a BSN and an active APN license, with RN licensure, and have 2 years of APN experience in hospice or home health, you will be part of a mission-driven team that values quality, compliance, and compassionate service.

Qualifications

• Graduate of a BSN from an accredited program.
• Current RN license and Advanced Practice Nurse license.
• Valid driver's license with reliable transportation.
• Two years APN experience in clinical care, hospice or home health preferred.
• Knowledge of pharmacology, palliative care and end-of-life care.

Responsibilities

• Educate patients and families about disease, self-care techniques, and end-of-life care.
• Assist in IDG's care plan development to ensure quality care.
• Collaborate with attending and hospice physicians on palliative care.
• Notify team of changes in patient needs and outcomes.
• Submit all documentation in a timely manner.
• Perform other duties as required.
