Heart to Heart Hospice is seeking a skilled Nurse Practitioner to deliver compassionate, evidence-based palliative care in the Plano area. You will partner with patients and families to understand goals, manage symptoms, and support comfort at end of life.

The role requires licensing as RN and APN, a BSN, a valid driver's license, and at least two years of APN experience in hospice or home health. You will collaborate with the IDG and physicians and ensure timely documentation and compliance.

Competenze

Graduate of a Baccalaureate School of Nursing.
Current RN and APN state licenses.
Valid driver's license and auto liability insurance.
At least two years APN experience in clinical care (hospice/home health preferred).
Knowledge of pharmacology, palliative care, and end-of-life care.

Mansioni

Educate patient/family about disease process and end-of-life care.
Assist in IDG's development of patient/family plan of care to assure quality care.
Collaborate with attending and consulting physicians for palliative care as needed.
Inform physicians and staff of changes in patient needs and outcomes; evaluate response to care.
Submit documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies.
