• Perform and document face-to-face encounters and home visits as part of palliation and end-of-life care.
• Provide education to patients and families about disease processes, self-care, and plan of care.
• Coordinate with attending physicians and hospice staff to ensure quality patient care.
• Support the IDG/IDT with timely communication and resource coordination.
