Optum Home & Care is hiring HouseCalls Advanced Practice Clinicians (NPs or PAs) to conduct annual in-home health assessments for Medicare Advantage members. You will educate patients, coordinate with primary care providers, and close gaps in care while supporting overall outcomes.

This field-based role emphasizes collaboration, documentation, and patient education, with a focus on safe, effective home visits and comprehensive care planning.

Qualifications

• In-home assessments and care management experience.
• Ability to coordinate with PCPs and health teams.
• Strong communication and documentation skills.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare members.
• Document diagnoses and develop care plans with PCPs.
• Educate patients on conditions, meds, and adherence.
• Address social determinants of health and provide referrals.
• Identify urgent situations and intervene appropriately.
