Relode is seeking a Nurse Practitioner for home health to lead clinical care for patients served with Medicare. The role includes extensive driving, in-home house calls, and longitudinal management of patient conditions with an emphasis on quality and patient satisfaction.

The position offers a starting salary around $115,000, with a $7,500 sign-on bonus and full medical/dental/vision benefits, PTO, and 401K—reflecting a mission to transform Medicare experiences through value-based care.

Qualifications

• Master's degree or higher in nursing (NP).
• 1 year of home health experience.
• Value-based care experience preferred.
• Experience working in a startup preferred.

• Comprehensive management of patient conditions: assessment, diagnosis, treatment, and longitudinal care.
• Support transitions of care via collaboration with post-acute care facilities.
• Accurately document patient conditions and encounters in an EMR.
• Ensure high quality and patient satisfaction with every encounter.
• Collaborate with care team to address social determinants impacting care.

As the lead nurse practitioner, you will influence and ensure the best clinical care for your patients.

Pay:

Starting at $115,000 Based on experience

$7,500 Sign-on Bonus

Requirements:

Masters' degree or higher

1 year of home health experience

Value-based care experience (preferred)

Experience working in a start-up (preferred)

Job Duties:

Lots of driving, In-home house calls (80% of the time)

Comprehensive management of patient conditions, including assessment, diagnosis, treatment, and consultation, on a longitudinal basis.

Transitions of care support via collaboration with post-acute care facilities.

Accurately document patient conditions and encounters in an EMR.

Ensure high levels of quality and patient satisfaction with each encounter.

Partner with the care team to identify and eliminate social determinants of healthcare for your patients, including functional capacity, home environment, health literacy, housing instability, food insecurity, and caregiver support.
