Home Health - Advanced Practice Provider -- Full Time
The client seeks Nurse Practitioners or Physician Assistants to provide home-based care for patients with chronic illnesses, especially kidney disease.
Roles and Responsibilities
- Conduct assessments, including comprehensive annual wellness exams in patients’ homes and via virtual visits.
- Counsel and educate patients and families about benefits, programs, and healthy living.
- Document patient encounters, including chief complaint, diagnoses, history, review of systems, examination, medications, allergies, assessment, and plan.
- Coordinate care with primary providers, specialists, and ancillary services.
- Complete all documentation and paperwork promptly.
- Maintain quality of care standards defined by the practice.
- Provide remote care; attend periodic training or meetings outside the state of employment.
- Deliver evidence-based care to reduce avoidable hospitalizations and maximize quality of life.
- Prescribe medications, order tests, and collaborate with the client physician.
- Measure and report improved patient outcomes daily.
- Facilitate preventive monitoring and coordinate diagnostic testing.
- Assist patients in enrolling for educational videos.
- Participate in integrated care team meetings.
- Maintain competency in disease diagnosis and prevention.
- Perform patient health status assessments.
- Implement care plans.
- Follow up and evaluate patient status.
- Other duties as assigned.
Position Requirements
- Active, unrestricted RN and NP or PA license.
- Board certified for the appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current, unrestricted DEA certificate.
- Ability to work autonomously without direct supervision.
- Valid driver’s license, car insurance, and reliable transportation.
- Proficiency with required medical instruments and equipment.
- Knowledge of computer-based data management programs and medical record systems.
- Excellent verbal and written communication skills.
- Understanding of federal and state regulations, including HIPAA and OSHA.
- Minimum 2 years direct patient care.
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
