Qualifications

• Minimum of 1 year of home health experience.
• Preferred value-based care experience.
• Experience working in a start-up is preferred.

Responsibilities

• Conduct in-home house calls (80% of the time).
• Manage patient conditions through assessment, diagnosis, and treatment.
• Collaborate with post-acute care facilities for transitions of care.
• Document patient conditions and encounters in EMR.
• Ensure high levels of quality and patient satisfaction.
