Job summary

UnitedHealth Group’s Optum Home & Community Care seeks a HouseCalls Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other plans. This field-based, non-prescribing role focuses on preventive care, member education, and closing care gaps with primary care providers.

Responsibilities include comprehensive home visits, medication reconciliation, vitals and screenings, and identifying social determinants of health with referrals as needed.

Qualifications

• ANCC or AANP certification in Family, Adult, Geriatric or Adult-Gerontology Primary Care.
• Active, unrestricted state licensure or ability to obtain by start date.
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to navigate a variety of home environments during assessments.
• Meet state requirements for independent practice approvals where required.

Join Optum Home & Community Care as a HouseCalls Nurse Practitioner and provide personalized, preventive care directly in members' homes.

This role offers flexibility and work-life balance, with providers able to flex their schedules between 8 a.m. and 8 p.m. across the 7-day work week. There are no on-call, weekend or holiday requirements.

As a HouseCalls Nurse Practitioner, you'll conduct annual in-home health assessments for Medicare Advantage and other plan members. This is a field-based, non-prescribing role focused on preventive care, member education, identifying gaps in care and collaborating with primary care providers.

Primary Responsibilities

• Conduct comprehensive in-home health assessments

Preferred Qualifications

• 1+ years of clinical experience, preferably in family, geriatric or home health care
• Experience working with geriatric or Medicare populations
• Proficiency with electronic medical records and technology
• Ability to transport equipment weighing up to 30 pounds and navigate stairs during home visits
• Strong communication skills
