Job summary

Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in the Altamonte Springs, FL clinic. In collaboration with the local physician team, APPs develop plans of care, provide diagnostics and treatments in clinic, home, or telehealth, and educate patients and families on medication usage, diet, and prevention.

Ideal candidates will have primary care experience and thrive in a growing value-based care environment.

Qualifications

• Master’s degree in nursing is required.
• Board Certification in Adult, Adult-Geriatric or Family NP is required.
• DEA registration or eligibility is required.
• BLS certification is required.
• Experience providing care in patients’ homes is preferred.

Responsibilities

• Develop and implement clinical plans of care for adult patients with chronic conditions.
• Gather patient history, perform physical exams, and diagnostics in home or clinic.
• Develop and implement treatment plans aligned with patient goals and conditions.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Provide compassionate care for complex patients and families.
• Educate patients and families about medications, nutrition, and crisis prevention.
