Job summary

AlignMed Partners seeks a full-time Advanced Practice Provider (NP or PA) in Epsom, NH to deliver primary and urgent care to nursing center residents. The role works with the Center Leadership Team and Medical Director to provide patient-centered care, leveraging collaboration across clinicians.

Qualifications

• Graduate from an accredited nursing program and be licensed as a registered nurse with NP certification (Older Adult and/or Family NP).
• Physician Assistant must be ARC-PA accredited and hold PA National Certifying Exam (PANCE) with valid state licensure.

Responsibilities

• Provide primary and urgent care services to nursing center residents.
• Collaborate on-site with Center Leadership and Medical Director for patient care.
