Job summary

Epic Healthcare is seeking a compassionate Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility. The NP will collaborate with physicians, nurses, and the interdisciplinary team to ensure high-quality medical care for residents.

Responsibilities include resident assessments, chronic condition management, timely responses to changes in condition, ordering labs, and preparing care plans.

Qualifications

• Active New Jersey Nurse Practitioner license.
• Board Certified NP with FNP/AGNP/GNP preferred.
• Experience in SNF / long-term care / geriatrics preferred.
• Proficient with PCC or ECW.
• Strong clinical decision-making and communication.
• Ability to work collaboratively with nursing home staff.

Responsibilities

• Conduct routine and acute patient assessments for nursing home residents
• Manage chronic conditions and develop treatment plans
• Respond to changes in residents’ conditions and provide timely care
• Order and interpret diagnostic tests and labs
• Prescribe medications in accordance with New Jersey regulations
• Coordinate care with physicians, nursing staff, and families
• Ensure proper documentation in the EMR
• Participate in care plan meetings and regulatory compliance
