Job summary

Impact Health PC in North Haven, CT seeks a Nurse Practitioner (NP) to provide comprehensive medical/primary care to residents in skilled nursing facilities. You will assess and manage acute and chronic conditions, create individualized treatment plans, and collaborate with physicians and interdisciplinary teams.

The role offers flexible scheduling with no nights, weekends, or holidays and strong clinical support, including physician-level consult access.

Qualifications

• Active CT NP license or credential.
• Minimum 2 years post-certification clinical experience preferred.
• Experience in skilled nursing facilities, primary care, hospitalist medicine, ED or ICU preferred.
• Proficient with EHRs and clinical technology.

Responsibilities

• Provide medical/primary care to residents in skilled nursing facilities.
• Assess, diagnose and manage acute and chronic conditions.
• Order labs, imaging, medications, and document orders.
• Collaborate with physicians, nursing staff, specialists and interdisciplinary teams.
• Educate patients and families on treatment plans.
