Job summary

Medical Director Services PC is seeking an experienced Nurse Practitioner (NP) to deliver high-quality primary care in a skilled nursing facility. New grads are welcome, with an excellent salary commensurate with experience.

The NP will provide comprehensive health assessments, manage acute and chronic conditions, collaborate with physicians and nursing staff, and educate residents and families to plan evidence-based care aligned with patient preferences.

Qualifications

• Must be Board Certified Nurse Practitioner.
• Experience in a skilled nursing facility setting is preferred.
• Demonstrates strong clinical judgement and teamwork with medical staff.

Responsibilities

• Serves as primary care provider for long- and short-term residents.
• Collaborates with medical attending physician and nursing staff.
• Educates residents and families and formulates evidence-based care plans.
• Ensures compliance with applicable laws and regulations and advises management as needed.
• Maintains professional knowledge through ongoing education and professional involvement.
• Maintains safe, clean working environment and upholds organizational values.
• Reviews charting and care documentation for timeliness and accuracy.
