Job summary

Apploi is seeking a compassionate Nurse Practitioner to provide primary and ongoing care in a skilled nursing facility. You will work with physicians, nurses, and the interdisciplinary team to ensure residents receive high-quality medical care.

Responsibilities include comprehensive assessments, managing chronic conditions, ordering tests, and documenting in the EMR. A NJ NP license and board certification (FNP/AGNP/GNP) are preferred, with a collaborative, patient-centered approach.

Qualifications

• Active New Jersey NP license.
• Board certification as NP; FNP/AGNP/GNP preferred.
• Experience in SNF/long-term care/geriatrics preferred.
• Strong clinical decision-making and communication skills.
• Ability to work collaboratively with nursing home staff.

Responsibilities

• Conduct routine and acute assessments for nursing home residents.
• Manage chronic conditions and develop treatment plans.
• Respond to changes in residents' conditions and provide timely care.
• Order and interpret diagnostics and labs.
• Prescribe medications per NJ regulations.
• Coordinate care with physicians, nursing staff, and families.
• Document in the EMR.
• Participate in care plan meetings and regulatory compliance.
