Job summary

Epic Healthcare is seeking a compassionate Nurse Practitioner (NP) to provide primary and ongoing care to residents in a skilled nursing facility in New Jersey. The NP will collaborate with physicians, nurses, and the interdisciplinary team to ensure high‑quality medical care for residents.

Responsibilities include conducting routine assessments, managing chronic conditions, ordering tests, prescribing medications per NJ regulations, and coordinating care with families and staff.

Qualifications

• Active New Jersey NP license is required.
• Board certification in an NP specialty preferred (FNP/AGNP/GNP).
• Experience in skilled nursing facility, long-term care, or geriatrics preferred.
• Strong clinical decision-making and communication skills are essential.

Responsibilities

• Perform routine and acute assessments of nursing home residents.
• Develop and manage treatment plans for chronic conditions.
• Respond promptly to changes in residents’ conditions with timely care.
• Order and interpret diagnostic tests and labs.
• Prescribe medications per New Jersey regulations.
• Coordinate care with physicians, nursing staff, and families.
• Document accurately in the EMR and maintain chart integrity.
• Participate in care plan meetings and ensure regulatory compliance.
