Job summary

The role includes performing routine and acute assessments, managing chronic conditions, and developing individualized treatment plans. Proficiency in PCC or ECW is preferred, and licensure requirements must be met.

Qualifications

• Active New Jersey Nurse Practitioner license is required.
• Board Certification as NP (FNP, AGNP, or GNP) is preferred.
• Experience in skilled nursing facility, long-term care, or geriatrics is preferred.
• PCC or ECW proficiency is expected.
