Job summary

Epic Healthcare in New Jersey is seeking a compassionate Nurse Practitioner to provide primary and ongoing care to skilled nursing facility residents. The NP will collaborate with physicians, nurses, and the interdisciplinary team to deliver high-quality medical care.

The role includes performing routine and acute assessments, managing chronic conditions, and developing individualized treatment plans. Proficiency in PCC or ECW is preferred, and licensure requirements must be met.

Qualifications

• Active New Jersey Nurse Practitioner license is required.
• Board Certification as NP (FNP, AGNP, or GNP) is preferred.
• Experience in skilled nursing facility, long-term care, or geriatrics is preferred.
• PCC or ECW proficiency is expected.

Responsibilities

• Conduct routine and acute patient assessments for nursing home residents.
• Manage chronic conditions and develop treatment plans.
• Respond to changes in residents’ conditions and provide timely care.
• Order and interpret diagnostic tests and labs.
• Prescribe medications in accordance with New Jersey regulations.
• Coordinate care with physicians, nursing staff, and families.
• Ensure proper documentation in the EMR.
• Participate in care plan meetings and regulatory compliance.
