Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive primary care to seniors across skilled nursing, assisted living, and independent living communities. The role emphasizes autonomy, strong patient relationships, and collaboration with physicians and care teams.

You will travel between facilities, build trusted clinical relationships, and use advanced technology to improve outcomes while supporting care plans and preventative strategies.

Qualifications

• Valid nurse practitioner license in state of operations and current board certification
• Master's Degree in Nursing or PA studies and active certification required
• Strong leadership, interpersonal, and problem-solving skills; ability to work autonomously with clinicians and operations teams

Responsibilities

• Deliver comprehensive primary care to residents in skilled nursing, assisted living, and independent living communities
• Diagnose, treat, and manage acute and chronic conditions with focus on preventative care
• Develop individualized treatment plans and provide ongoing care management
• Build strong patient relationships and conduct facility rounds with care teams
• Collaborate with physicians, clinical staff, and family members to coordinate care
• Maintain accurate EMR documentation and ensure regulatory compliance
