Job summary

Curana Health seeks compassionate Nurse Practitioners or Physician Assistants to deliver high-quality primary care in senior living facilities. You will provide direct patient care, manage chronic conditions, and coordinate with other healthcare providers to improve health outcomes.

This full-time role includes travel to assigned facilities, emphasis on wellness promotion, and a collaborative team environment with strong clinical and administrative support.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification
• Masters Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills
• Must possess excellent written and oral communication skills

Responsibilities

• Perform comprehensive assessments and develop treatment plans for residents
• Conduct routine rounds and monitor residents' conditions
• Coordinate care with facility staff, families, and other providers
• Educate residents and families on disease prevention and wellness
• Document accurately in EMR and ensure regulatory compliance
• Participate in quality improvement initiatives to enhance care
