Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care to seniors in our care communities. You will practice with autonomy, supported by physicians, operational leaders, and advanced technology to improve outcomes and reduce hospitalizations.

You will build strong patient relationships, conduct facility rounds, and coordinate care with the care continuum.

Qualifications

• Masters Degree in Nursing or Physician Assistant Studies is required.
• Strong leadership, interpersonal, and problem-solving skills are preferred.
• Excellent written and oral communication skills are essential.
• Punctuality, organizational skills, and ability to function within a team are required.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Build meaningful patient relationships and conduct regular facility rounds with care teams.
• Collaborate across the care continuum with physicians, staff, and families to coordinate care.
• Drive clinical excellence by improving outcomes and maintaining accurate EMR documentation.
