Curana Health is seeking a Nurse Practitioner or Physician Assistant to deliver on-site primary care within senior living communities and skilled nursing facilities. You will practice with autonomy and partner with physicians, care teams, and facility leadership to optimize patient outcomes.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Current board certification in your field.
• Strong leadership, interpersonal, and communication skills.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Build meaningful patient relationships and coordinate care with physicians, clinical staff, facility leadership, and families.
• Maintain accurate and timely documentation within the EMR and ensure regulatory/organizational standards are met.
