UnitedHealth Group through Optum HouseCalls is seeking an Advanced Practice Clinician (NP/PA) to conduct in-home health assessments for Medicare Advantage and other plan members. The role focuses on education, gap closure, and collaboration with primary care providers to improve outcomes, in a home-based setting.

This non-prescribing, field-based position offers flexible scheduling with no weekends or holidays, and a compensation package with base pay and bonus opportunities, along with

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If required by state, obtain licensing board approval within specified timelines.

Responsibilities

• Conduct comprehensive in-home assessments, including past medical history review, medication reconciliation, vital signs, and physical exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
