Job summary

Optum Home & Community Care is seeking a Part-Time Nurse Practitioner to conduct comprehensive in-home assessments in Eureka, IL. Enjoy flexible scheduling with coverage from 8am–8pm across a 7-day week, no on-call, weekends, or holidays, and a $10,000 sign-on bonus for external candidates.

You will identify diagnoses, document care plans, coordinate with PCPs, educate members, and address social determinants of health while contributing to a multidisciplinary team.

Qualifications

Active ANCC or AANP Family/Adult/Geriatric Primary Care certification.
Current unrestricted license to practice in the state.
Driver's license and reliable transportation for home visits.
Ability to perform comprehensive in-home assessments.
Willingness to practice with varied home environments.

Responsibilities

Perform in-home assessments including PMH, med reconciliation, vitals, and exam.
Document diagnoses for care management and planning.
Communicate findings to PCPs to address gaps in care.
Recognize urgent situations and intervene as needed.
Educate members on disease processes, meds and adherence; address social needs.
