Job summary

Optum Home & Community Care, part of the Optum family, is expanding its HouseCalls program with in-home assessments conducted by a Nurse Practitioner or Physician Assistant. This non-prescribing, field-based role improves health outcomes through education, gap closure, and collaboration with PCPs.

Flexible scheduling within 8am-8pm across a 7-day week; no weekends or holidays; remote work not required as visits are in patients’ homes in Missouri.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in Missouri (or ability to obtain by start date) without current disciplinary actions or practice limitations.
• Active and unrestricted driver's license and access to reliable transportation to complete home visits.
• Ability to navigate varied home environments; ability to sit/stand/kneel as needed to perform assessments.

Responsibilities

• Perform comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Identify urgent/emergent conditions and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
