Job summary

Trio Health Partners is seeking an In-House Nurse Practitioner to serve as the primary internal medicine provider within a Skilled Nursing Facility. You will build relationships with residents, families, facility staff, and other providers while contributing to clinical leadership.

The role focuses on proactive geriatric care, preventive medicine, and reducing hospitalizations in skilled nursing and long-term care facilities, with a strong emphasis on team collaboration and resident well-being.

Qualifications

  • Active Illinois NP license (unencumbered)
  • Master's Degree in Nursing from an accredited program
  • Strong clinical background in primary care and preventive medicine
  • Geriatric or adult medicine experience in SNF/long-term care settings preferred
  • Ability to engage elderly and chronically ill patients and families
  • Able to achieve ≥12 patient visits per day with bonuses for exceeding this target

Responsibilities

  • Conduct new admission evaluations and monthly stability visits for long-term residents
  • Perform comprehensive physical examinations and monitor changes in patient condition
  • Prescribe medications and treatments as needed
  • Develop patient care policies and facilitate referrals to specialists
  • Coordinate advanced care planning and discharge visits
  • Ensure timely documentation of patient assessments, treatment plans, and follow-ups
  • Support the physician-patient relationship and address gaps in care

As anIn-House Nurse Practitioner, you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:
  • Active Illinois Nurse Practitioner License (unencumbered).
  • Master's Degree in Nursing from an accredited program.
  • Strong clinical background in primary care and preventive medicine.
  • Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
  • Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
  • Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
  • Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.
Key Responsibilities:

  • Annual Pay: $105,000 - $130,000 based on experience
