Job summary

Trio Health Partners is seeking an in-house Nurse Practitioner to lead internal medicine services within a Skilled Nursing Facility. You will build lasting relationships with residents, families, facility staff, and other providers while contributing to clinical leadership.

The role emphasizes proactive geriatric care, preventive medicine, and collaboration to prevent hospitalizations in long-term care settings.

Qualifications

• Active Illinois NP license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with focus on skilled nursing/long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Responsibilities

• Conducting new admission evaluations and monthly stability visits for long-term residents.
• Performing comprehensive physical examinations and evaluating changes in patient conditions.
• Prescribing medications and treatments as needed.
• Developing patient care policies and facilitating referrals to specialists.
• Coordinating advanced care planning and discharge visits.
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups.
• Supporting the physician-patient relationship and addressing gaps in care.

Job Requirements

• Annual Pay: $105,000 - $130,000 based on experience
• Monthly Bonus Opportunities: Monthly performance-based bonuses
