Job summary

Trio Health Partners is seeking an In-House Nurse Practitioner to serve as the primary internal medicine provider in our Skilled Nursing Facility. You will build relationships with residents, families, and staff while contributing to the clinical leadership team.

The role emphasizes proactive geriatric care, preventing hospitalizations, and delivering comprehensive primary care with a focus on long-term resident well-being.

Qualifications

• Licensed Nurse Practitioner with a Master's in Nursing.
• Active Illinois NP license.
• Strong clinical background in primary care and preventive medicine.

Responsibilities

• Conduct new admission evaluations and monthly stability visits for long-term residents.
• Perform comprehensive physical examinations and monitor changes in patient conditions.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.

As anIn-House Nurse Practitioner, you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities:

• Conducting new admission evaluations and monthly stability visits for long-term residents
• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care

Salary & Compensation:

• Annual Pay: $105,000 - $130,000 based on experience
• Monthly Bonus Opportunities: Monthly performance-based bonuses
