Job summary

Trio Health Partners in Illinois is seeking an In-House Nurse Practitioner to serve as the primary internal medicine provider within a Skilled Nursing Facility. You will build strong relationships with residents, families, facility staff, and providers while contributing to clinical leadership.

The role emphasizes preventive care, coordination of care, and adherence to Medicare/Medicaid guidelines, with a schedule that avoids nights and weekends and offers a monthly bonus and comprehensive

Qualifications

• Strong clinical background in primary care and preventive medicine.
• Experience in geriatric or adult medicine settings preferred.
• Ability to engage with elderly patients and families effectively.

Responsibilities

• Provide essential in-house internal medicine care within the SNF.
• Conduct admissions evaluations and monthly stability visits.
• Prescribe medications and treatments as needed.
• Develop patient care policies and coordinate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely documentation of assessments and treatment plans.
• Support the physician-patient relationship and address gaps in care.

As anIn-House Nurse Practitioner, you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities

As the In-House Nurse Practitioner, you will provide essential care and leadership within the facility, including:

• Conducting new admission evaluations and monthly stability visits for long-term residents
• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care

Salary & Compensation

• Annual Pay: $105,000 - $130,000 based on experience
• Monthly Bonus Opportunities: Monthly performance-based bonuses
