Job summary

TRIO HEALTH PARTNERS seeks an In-House Nurse Practitioner to deliver internal medicine services within a Skilled Nursing Facility, building lasting relationships with residents, families and staff. You will be a clinical leader in a team dedicated to proactive geriatric care.

Qualifications

• Active Illinois NP license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience in skilled nursing facilities.
• Ability to engage and communicate effectively with elderly patients and families.
• Minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare/Medicaid and Health Plan benefit structures is a plus.

Responsibilities

• Conduct new admission evaluations and monthly stability visits for long-term residents.
• Perform comprehensive physical examinations and evaluate changes in patient conditions.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely communication and documentation of assessments, treatment plans, and follow-ups.
• Support the physician-patient relationship and address gaps in care.

As anIn-House Nurse Practitioner, you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements

• Active Illinois Nurse Practitioner License (unencumbered).

• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities

As the In-House Nurse Practitioner, you will provide essential care and leadership within the facility, including:

• Annual Pay: $105,000 - $130,000 based on experience
• Monthly Bonus Opportunities: Monthly performance-based bonuses
