Qualifications
- Active nurse practitioner or physician assistant license in the state of operations
- Current board certification in your field
- Strong leadership, interpersonal, and problem-solving skills
- Excellent written and oral communication and organizational abilities
Responsibilities
- Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities
- Perform comprehensive health assessments and develop individualized treatment plans
- Diagnose, treat, and manage acute and chronic conditions with focus on prevention
- Coordinate care with physicians, facility leadership, and care teams to improve outcomes
- Maintain accurate EMR documentation and ensure regulatory compliance
