Circadia Health is seeking a Nurse Practitioner to join our on-site team, covering Dover and Bedford, NH. You will blend direct patient care with real-time AI-driven health signals, coaching facility staff to intervene earlier and reduce readmissions.

As a full-time NP, you will see patients first, interpret trend signals, and train teams to integrate Circadia into clinical workflows. Strong autonomous practice and travel within NH are expected.

Qualificações

• Certified and licensed Nurse Practitioner in NH; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid NH driver's license; comfortable traveling locally.
• Thrives in a dynamic startup environment.
• English required

Responsabilidades

• Direct patient care: conduct initial and follow-up assessments on flagged residents; adjust plans of care; coordinate with facility providers.
• Tech to bedside integration: train and coach staff; standardize Circadia usage in workflows.
• Data into action: interpret trends, prioritize high-risk patients, drive timely interventions.
• Close the loop: complete documentation within the same day.
• High-volume rounding: 30+ visits/day; focus on new admits and Circadia alerts.
• Quality & product feedback: surface field insights to improve workflows and integration.
• Mentor & collaborate: support NP onboarding; partner with clinical ops/product teams.
