Circadia Health in Bangor, ME seeks a Nurse Practitioner to lead AI-assisted patient care across skilled nursing facilities and home health programs. You will act on predictive signals in real time, coach facility teams, and blend direct care with tech-to-bedside integration.

Ideal candidates hold ME NP licensure with experience in complex adult populations, travel locally, and thrive in a dynamic startup setting.

Qualifications

Certified and licensed Nurse Practitioner in ME with ANCC or equivalent.
Eligible for Medicare participation and able to bill on your behalf.
Minimum 2+ years NP experience with complex adult populations and independent practice.
Valid ME driver’s license and comfortable traveling locally.
Thrives in a dynamic startup environment and uses English professionally.

Responsibilities

Direct patient care: initial and follow-up assessments; plans of care; consent; coordination with facilities.
Tech to bedside integration: train staff and standardize Circadia use in clinical workflows.
Interpret trend‑based insights; prioritize high‑risk patients; drive timely interventions.
Complete documentation within the same day.
Rounding across SNFs and home health visits with Circadia alerts.
Provide mentorship for NP onboarding and collaborate with clinical ops/product teams.
Cover SNF sites and patient homes within a 30–40 mile radius; mileage covered.
