Job summary

Circadia Health is seeking a Nurse Practitioner to deliver direct patient care across SNF and Home Health AI programs. You’ll see patients first, act on predictive signals in real time, and coach facility teams to turn data into earlier interventions that reduce readmissions.

You’ll blend clinical work with tech-to-bedside integration, receive hands-on training and live demonstrations, and support NP onboarding while traveling within a 30–40 mile radius with mileage covered by company policy.

Qualifications

NJ NP license and ANCC credentials.
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations.
Valid NJ driver’s license; comfortable traveling locally.
Thrives in a dynamic startup environment.
English required.

Responsibilities

Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
Close the loop: Complete documentation/charts within the same day.
High‑volume rounding: 25+/day visits across SNFs when appropriate.
Mentor & collaborate: Support NP onboarding and partner with clinical ops/product teams.
