Job summary

Circadia Health is seeking a Nurse Practitioner to deliver direct patient care in skilled nursing facilities and home health settings. You’ll act on real-time predictive health signals and coach facility teams to intervene earlier to reduce readmissions.

In SNF settings you’ll round on new admits and flagged patients; in home health you’ll extend high-quality care to patients at home, blending clinical work with tech-enabled workflows.

Qualifications

• Certified and licensed NP in PA.
• 2+ years NP experience with complex adult populations.
• Eligible for Medicare participation and able to practice independently.

Responsibilities

• Provide direct patient care across SNF and home health.
• Integrate Circadia tech into clinical workflows at the bedside.
